Depending on your insurance plan, you may be responsible for:

• Services requiring prior authorization
• Any applicable self-pay balance

Your insurance company determines your individual benefits and financial responsibility. Verification of benefits does not guarantee that your insurance company will pay for a service.

We also offer self-pay options for patients who do not have insurance or choose not to use their insurance.
